Corrective action: Person in charge discarded the items during the inspection. See voluntary condemnation.
4-601.11 — (A) Food contact surfaces were not clean to sight and touch.
Corrective action: Inspector observed the pizza dough spreader with excess dough and oil not cleaned after use or every four hours. Person in charge was instructed to clean the equipment at a greater frequency.
3-501.16 — (A)(2) Food item(s) that require temperature control for safety were cold-holding at internal temperatures greater than 41°F.
Corrective action: Inspector observed several TCS foods holding between 56-61°F for longer than four hours in the cold holding units across from the hot food prep line. Person in charge discarded the items during the inspection. See voluntary condemnation.

Corrective action: Inspector observed Bacon prepared on 06/12/2026, Deli ham prepared on 06/13/26, Chicken prepared on 05/09/26, and Salsa prepared on 06/15/2026 that had not been disposed of after 7 days. Person in charge discarded the items during the inspection. See voluntary condemnation.
4-301.11 — The equipment available is not capable of cooling, heating, or holding foods at the temperatures required by the code.
Corrective action: Inspector observed the cold holding units across from the hot food prep line holding the food inside at an ambient temperature of 56-61°F. Person in charge will submit a work order to have this equipment repaired.
6-202.15 — Inspector observed several areas of heavy mouse fecal deposits throughout the facility, as well as many live flies.
Corrective action: Inspector discussed the frequency of cleaning up the mouse droppings, sealing the outer openings, and keeping a more regular pest control service. Person in charge was instructed to deep clean the areas with mouse droppings present and to keep their screen closed to prevent the entry of pest.
3-303.12 — Inspector observed water from a faulty cold holding unit dripping on top of cups of cheese that were stored inside.
Corrective action: Person in charge discarded the cheese and will submit a work order to have the equipment repaired.
4-602.13 — Nonfood contact surfaces of equipment were not kept free of an accumulation of dust, dirt, food residue and other debris.
Corrective action: Inspector observed heavy dust accumulation on the fans, ceiling vents, and refrigeration equipment screens. Person in charge was instructed to clean this equipment at greater frequency to prevent the circulation of dust and debris in the kitchen, as well as increase the functionality of the cooling equipment.
