Corrective action: Observed employee changing tasks when handling the register, picking up items off the floor, and ware washing without hand washing. Education offered on when to wash requirements with person in charge.
5-205.11 — Hand washing sinks were observed being used for activities that were not hand washing.
Corrective action: Observed employee stack rinsed blenders in the basin of the hand-washing sink during the cleaning process. Education offered on hand sink usage requirements and blenders were moved during inspection.

Observed blenders and utensils held out of temperature control used with TCS foods only rinsed between each use and cleaned every five hours. Spoke with person in charge about ensuring equipment in contact with TCS foods are adequately washed, rinsed and sanitized at least every 4 hours.

Corrective action: Observed lettuce and kale in prep coolers holding between 43-50°F for less than four hours per person in charge. Doors were kept closed for these prep coolers and temperatures were observed to decline.
7-204.11 — Poisonous or toxic materials were not used in accordance with manufacturer's directions.
Corrective action: Observed chlorine spray bottle testing above 200 ppm by the register that was reportedly used for kitchen cleaning. Reviewed sanitizer concentration requirements with person in charge during inspection, this solution will be used for bathroom cleaning only.

Corrective action: Observed employee did not change gloves and wash hands when changing tasks at the smoothie station. Management staff educated employee and hands were washed and gloves were changed during inspection.

Corrective action: Observed a box containing food stored on the floor of the walk-in freezer.
4-501.11 — Utensils or equipment were not maintained in good repair or condition.
Corrective action: Observed the cold-holding prep cooler in need of service and repair. Person in charge placed a work order for service and repair during inspection.
