Corrective action: Observed employee was hands for less than 20 seconds after handling dirty dishes. Informed person in charge that hands must be washed for at least 20 seconds with 10-15 seconds used to scrub hands with soap and water.
None — 15: 3-302.11(A)(3)-(8) Observed multiple containers of uncovered ready to eat food stored in the reach in refrigerators. Informed person in charge that food must be protected from contamination by storing the food in covered container or wrappings.
None — 16: 4-602.11(C) Equipment, utensils, and/or food-contact surfaces used in contact with food items that require time/temperature control for safety are not being cleaned at least every 4 hours. Was informed by person in charge that in use utensils and cutting boards are switched out/cleaned every 6 hours. Informed person in charge that the time frame for equipment used with time/temperature control foods is every 4 hours. Cutting boards were flipped to the clean sanitized side during the inspection.

Corrective action: The menu was observed with a consumer advisory but lacking a way of indicating the items that fall under the advisory. Person in charge will update the menu.
None — 36: 4-502.11(B) Food thermometers were not accurate within 2°F. When comparing temperatures on two different thermometers, there was temperature differences. Was informed by person in charge that the probe thermometers have never been calibrated. Person in charged learned how to calibrate the thermometers and calibrated it during the inspection.
None — 37: 3-602.11(B)(5) Food packaged in the establishment was not labeled as required by the code. Observed containers of spicy matcha on the dry storage shelves without any form of labeling. Did not observe any form of allergen notice during the inspection. Informed person in charge that food packaged in the establishment must be labeled to inform consumers of ingredients, the business, and any allergens. Guidance will be given to person in charge.
3-304.12 — In-use utensils were improperly stored (without the handle above the top of the food, not on a clean surface, not in running water, or not in water of at least 135°F).
Corrective action: Observed a cup being used as a scooper and stored in the container of uncooked rice. Informed person in charge that scoops must be stored in a way to protect against contamination (i.e. with handles stored above the food or stored on a clean surface).
