None — 3: 7.7.1(B)(1) Observed no cover on the outside sand box. I was informed that the sandbox has been uncovered for the last week.

None — 8: 7.7.1(A)(7) Observed green rugs ripped along the edges in the toddler room. Rugs shall be replaced or repaired.

None — 11: 7.8.1(C)(5) Children's water bottles are not being sent home to be washed and are not washed at the facility. Individual water bottles shall be washed, rinsed, and sanitized daily after use.

None — 21: 7.8.6(A)(4) Observed no laundry sanitizer for washing contaminated items.

None — 26: 7.9.4(A)(5) Toys in the toddler room are being cleaned weekly. Classroom teachers were informed toddler toys shall be cleaned daily.

None — 31: 7.10.1(C)(2) Observed toddler illness logs missing return dates for all entries. Discussed required documentation with classroom teacher.

7.11 — Observed staff in preschool room serve pizza with their bare hands.

None — 43: 7.11.4(E)(1-4) Observed children in the school age room touching pizzas and cucumbers in a container with their bare hands and staff not supervising food service. Cucumbers were discarded, classroom staff began using tongs for the cucumber and classroom staff served the remaining pizza with gloved hands. Discussed proper family style food service with classroom staff.

None — 49: 7.12.4(B)(5) Observed rips and tears in the center of the changing mat in the toddler room. Mat shall be replaced and in good repair.
