Person in charge is currently using a restricted use pesticide to prevent insects within the establishment. Discussed with the person in charge that these forms of pesticide should only be applied by a certified exterminator. Person in charge will hire a professional to treat the establishment.
3-302.12 — Working containers holding food or food ingredients that are removed from their original packages shall be identified with common name.
Corrective action: Observed glass containers holding sauce without a label identifying its contents. Discussed with the person in charge that labels should be added to containers holding food items as to prevent possible cross contact for customers who have allergens.
6-501.112 — Dead or trapped birds, insects, rodents or other pests were not removed from the establishment often enough to prevent accumulation, decomposition, or the attraction of pests.
Corrective action: Observed dead cockroaches throughout the facility. Discussed with the person in charge about increasing the cleaning frequency to remove dead pests. Pests were present in the establishment. Observed live cockroaches present in the facility in the mop sink storage area. Discussed with the person in charge that a professional exterminator needs to be hired to maintain the facility. The person in charge will hire an exterminator and provide an invoice once they have treated the facility.
6-501.12 — Physical facilities were not cleaned as often as necessary to keep them clean.
Corrective action: Observed buildup of debris around the baseboards of the establishment. Discussed with the person in charge about increasing the cleaning frequency to prevent the accumulation of pests.
