3-304.14 — Wiping cloths were not held in a reasonably clean sanitizer solution of appropriate concentration between use.
Corrective action: Inspector observed that the sanitizer solution in multiple wiping cloth buckets was below the proper concentration and had become visibly soiled. Person in charge changed the buckets during the inspection.
4-202.16 — Non-food contact surfaces were not designed to be easily cleanable.
Corrective action: Inspector observed that several floor tiles were missing in the food prep area of the kitchen. Person in charge discussed putting in a work order to have the floors repaired.

4-501.12 — 1) Food contact surfaces were not easily cleanable.
Corrective action: Inspector observed the cutting board on the food prep line with excessive scoring and deep cuts. Person in charge was instructed to sand down or replace the cutting board. Additionally, a piece of cardboard that was not easily cleanable was placed behind the ice machine lid to help keep the lid propped open. 2) Equipment components such as doors, seals, hinges, fasteners, and kickplates shall be kept intact, tight and adjusted. Inspector observed that the ice machine lid would not stay propped open as intended. Person in charge will tighten the bushing on the lid to properly adjust it.
4-601.11 — (B), and (C) 1) Nonfood contact surfaces of equipment were not kept free of an accumulation of dust, dirt, food residue and other debris.
Corrective action: Inspector observed excessive grease, dust, food debris and material buildup on the coolers and freezers between the food prep line and the dishwasher. Person in charge was instructed to increase the frequency of deep cleaning equipment. 2) Nonfood contact surfaces of equipment were not cleaned at a frequency necessary to prevent the accumulation of soil residues. Inspector observed excessive grease, dust, food debris and material buildup on the coolers and freezers between the food prep line and the dishwasher. Person in charge was instructed to increase the frequency of deep cleaning equipment.
6-501.12 — 1) Physical facilities were not maintained in good repair.
Corrective action: Inspector observed broken tiles in the food prep line that were not easily cleanable. Person in charge was instructed to replace the tiles. 2) Floors, walls and ceilings were not smooth, durable, easily cleanable and/or nonabsorbent. Physical facilities were not cleaned as often as necessary to keep them clean. Inspector observed excessive dirt, grease, debris, and other materials throughout the facility on the floors, walls and behind equipment. Person in charge was instructed to increase the cleaning frequency of these areas.
