Corrective action: Facility was retaining the purveyors packaging slip, and failed to retain the harvester's tag for 90 calendar days. Spoke to person in charge about retaining the white tag stapled to the container.
None — 15: 3-302.11(A)(1)-(2) Raw animal foods were not separated from ready-to-eat foods during storage, or preparation, to prevent cross-contamination. Observed raw shelled eggs stored above shredded cheese. Eggs were moved to the bottom shelf.

Corrective action: Observed cooked pork and green chile holding at 127-130°F in the steam table. Both items were reheated to 165°F and the stem table was turned up.

Corrective action: Observed multiple unlabelled foods stored in the walk-in freezer. Items were labelled.
6-202.13 — Insect control shall be installed so that they are not located over food preparation areas.
Corrective action: Observed fly strip installed above food preparation table. Device was relocated by the back door.
