6-501.112 — Dead or trapped birds, insects, rodents or other pests were not removed from the establishment often enough to prevent accumulation, decomposition, or the attraction of pests.
Corrective action: Observed a few dead cockroaches under the water heater in the back of the kitchen and under the food prep table. Spoke with person in charge about removing pests to prevent the attraction of new pests. Outer openings were not protected against the entry of pests. Observed visible light appearing through the back door of the kitchen. Spoke with person in charge about a weather stripping to ensure all door cracks are sealed and protected against the entry of pests.

Corrective action: Observed a few boxes of food items being stored on the floor of the walk in freezer. Spoke with person in charge about ensuring food items are kept at least 6 inches above the ground to prevent the risk on contamination.

Spoke with person in charge about ensuring items are fixed. Per person in charge work orders have been put in for all items.

Corrective action: Observed an excessive buildup of grease along the fryers and under the equipment in the kitchen area. Spoke with person in charge about cleaning frequency for this item. Per person in charge item will be cleaned on the weekends.
