2-201 — (2) All body artists shall update the training required in Section 2-201 (a), at a minimum, once per year.
Corrective action: Observed 2 artist with expired blood borne certificates. Certificates shall be updated.
2-201 — (3) All body artists shall successfully complete and maintain approved training in Basic First Aid.
Corrective action: Observed 2 artist with expired first aid certificates. First aid training shall be updated.
3-309 — Client consent form is missing required documented information.
Corrective action: Observed no documentation of birth certificates or guardian identification for all minor consent forms. Discussed required documentation with person in charge.
4-401 — All procedure areas and instrument cleaning areas shall have floors, walls and ceilings constructed of smooth, nonabsorbent and easily cleanable materials.
Corrective action: Outer openings shall provide protection against contamination from dust and other contaminants. Observed procedure area dividers constructed of fabric. Surfaces shall be made smooth, nonabsorbent and easily cleanable.
4-416 — In new or extensively remodeled body art establishments, a conveniently located utility sink or curbed cleaning facility with a floor drain and hot and cold water, shall be provided and used for the cleaning of mops or similar wet floor cleaning materials, and for the disposal of mop water or similar liquid wastes.
Corrective action: Mop sink has not been installed.
4-418 — (1) Sharps and Infectious/Regulated Waste must be handled in a manner consistent with §25-15-401, CRS.
Corrective action: Discarded sharps shall be disposed of in approved sharps containers. Observed three over full sharps containers in procedure area 4 and 5. Sharps shall be disposed of at an approved disposal facility.
9-904 — (5) Current manufacturer information, including but not limited to mill specification sheets, shall be available to verify that jewelry used in section 9-904 (4) of these regulations meets those standards.
Corrective action: Manufacturer information for all piercing jewelry was not available during inspection. Person in charge was informed of the required documents for jewelry.
