2-201 — (2) All body artists shall update the training required in Section 2-201 (a), at a minimum, once per year.
Corrective action: Observed 1 expired blood borne pathogen certificate. Bloodborne pathogens training certificate shall be up to date.
3-309 — Client consent form is missing required documented information.
Corrective action: Observed 1) all client consent forms missing manufacturer and lot number information of pigments/inks and instruments used. 2) Observed minor consent forms missing copy of documentation to indicate parentage or guardianship such as birth certificates.
4-408 — Utility sinks, instrument cleaning sinks and hand sinks shall be separate and must only be used for their designated purpose.
Corrective action: Facilities hand wash sink is being used to clean equipment before being sterilized in autoclave.
4-401 — All procedure areas and instrument cleaning areas shall have floors, walls and ceilings constructed of smooth, nonabsorbent and easily cleanable materials.
Corrective action: Outer openings shall provide protection against contamination from dust and other contaminants. Observed fabric curtain used as a divider in procedure area and unfinished wooden shelves along the walls in all procedure areas. All surfaces shall be made smooth and non absorbent.
8-803 — Packages will no longer be considered sterile six months after the date of sterilization Packages that have reached the expiration date established by the manufacturer or in the absence of such expiration date have reached a date equivalent to six months after the date of sterilization will no longer be considered sterile.
Corrective action: Observed several pieces of piercing jewelry and piercing equipment 6 months past the sterilization date.
8-804 — (7) 1.
Corrective action: Sterilizer monitoring shall be performed at least weekly (unless more frequent monitoring is specified by the manufacturer) by using a commercial biological (spore) monitoring system 2. All biological indicators shall be analyzed by a laboratory independent from the establishment 3. Biological indicator test results shall be maintained on the premises for a minimum of three (3) years and must be available for inspection at all times 4. The department may require the operator to submit copies of the weekly sterilizer monitoring results by mail, facsimile or in person. Observed no spore testing / sterilizer monitoring has been done and no test results available.
