Corrective action: Observed minor consent forms missing birth certificates. Discussed required documents with person in charge.
4-401 — All procedure areas and instrument cleaning areas shall have floors, walls and ceilings constructed of smooth, nonabsorbent and easily cleanable materials.
Corrective action: Observed procedure room #4 newly painted with flat finish black paint that is not smooth and easily cleanable. Discussed this with person in charge.
8-803 — Packages that have reached the expiration date established by the manufacturer or in the absence of such expiration date have reached a date equivalent to six months after the date of sterilization, or that have been otherwise compromised either in handling or storage will no longer be considered sterile.
Corrective action: Observed several expired pre sterilized packages of 12 gauge piercing needles in procedure area #5. Expired needles were disposed of in sharps container during inspection.
