2-401.11 — Employee beverages did not meet the requirements in the code.
Corrective action: Observed multiple employee drinks throughout the kitchen without a lid and stray. Person in charge was in formed to have a designated area for employee beverages and beverages must have a lid and straw. Employee beverages were moved to a designated area at time of inspection.
3-501.16 — (A)(2) Food item(s) that require temperature control for safety were cold-holding at internal temperatures greater than 41°F.
Corrective action: Observed red pizza sauce, sliced provolone cheese, chopped green chili, cut tomatoes, baby spinach, shredded mozzarella, and sliced Canadian bacon all temping between 43-49 degrees F in the pizza prep cold hold unit longer than 4 hours. Informed person in charge that time/temp-controlled foods must be kept at or below 41 degrees F for cold holding. All items listed above were discarded during the inspection, see voluntary condemnation.
3-501.18 — Refrigerated, ready-to-eat food(s) that require time or temperature control for safety was not discarded within 7 days of preparation or opening of product.
Corrective action: Observed 1) brown gravy and alfredo sauce in the cold unit by the reach in freezer, 2) cooked linguini noodles in the upstairs walk-in refrigerator and in the front-line cold unit, all products held passed the 7-day discard date. Observed an open container of heavy cream in the cold unit by the reach in freezer without a date mark. Informed person in charge that all products held over 7 days shall be discarded and any time/temp-controlled foods must be dated for day product was opened or day of discard. All outdated products were discarded during the inspection, and a date was added to the heavy cream. See voluntary condemnation.
3-603.11 — The consumer advisory in the menu did not meet the requirements in the code.
Corrective action: Observed a consumer advisory on the menu, but it failed to identify which items applied. Informed person in charge that there must be a way to indicate which items are potentially undercooked and fall under the consumer advisory.
7-102.11 — A working container storing poisonous or toxic material was found without a label identifying its contents.
Corrective action: Observed multiple unlabeled spray bottles of sanitizer behind the bar. Poisonous or toxic materials were not used in accordance with manufacturer's directions. Observed the sanitizer behind the bar area reading above 400ppm for a quaternary sanitizer. Sanitizer was re-made during the inspection and bottles were labeled.
6-202.15 — Outer openings were not protected against the entry of pests.
Corrective action: Observed gap on the bottom of the freight door. Person in charge was informed that doors must be tight fitting to protect against entry of pest.
3-305.11 — Food was not stored at least 6 inches above the floor.
Corrective action: Observed green chili and loafs of bread stored on the floor of the downstairs freezer. Informed person in charge that food must be stored at least six inches off the floor. Product was moved during the inspection.
4-501.11 — Utensils or equipment were not maintained in good repair or condition.
Corrective action: Observed the pizza prep cold hold unit with an ambient temperature of 46 degrees F. Informed person in charge that cold hold units must be kept at 41 degrees F or lower. Maintenance showed up to fix the unit during the inspection.
6-403.11 — Personal belongings were stored in an unapproved area.
Corrective action: Observed 1) employee drink stored on the pizza prep cold hold unit, 2) employee phone stored on the meat slicer. Informed person in charge that employee drinks must be in a designated area, away from food and clean equipment to prevent contamination.
